The SKIP Flu Immunization clinic through Public Health Solutions will be offered at school on Wednesday, October 11. To have your child vaccinated, please do the following:
1. Complete and sign the permission form. Forms will be sent home with students but are also available online at https://5il.co/269rc
2. Insured? - Please include a copy of your insurance or Medicaid/Medicaid Managed Care Cards.
3. Are you uninsured? Mark the uninsured box on the Permission Form.
